Lane change checklist

I think I saw on lessons the order of checks before changing lanes but I cannot find which lesson it was.
This is what I do:
1. Check rear mirror
2. Check side mirror.
3. If everything is O.K, Signal.
4. Check side mirror.
5. Check blind spot.
6. Slowly change lane (so that give others time to react if I made a mistake).

Is this correct?
