The begeleider is required to sign the application form

The municipality will fill in the details based on the ID/licence info you give them,
then you get the form to let the begeleider sign it @ home
(so you don't have to take the begeleider along to the municipality , though you can if it's convenient - it's faster that way)

But they'll also need proof of your begeleider's Vormingsmoment in Flanders ...

So, it's nothing you can quickly arrange between your practical exam and the expiry date of your provisional licence .

Oh i see, can i fix it on my own? As long i have the photocopy of id card, rijbewijs and his attest as official begeleider. Is my begeleider required to sign on the form (his signature)? The problem is i still need to take my praktijk exam on june 13 thats why just incase i dnt pass it ( i hope not🙏), after june 13 only i can request for extension of my rijbewijs then my begeleider (father in law) is not here in belgium the whole month of june coz he is in vacation. Thats why its a little bit conflict for me...

You absolutely have to change it before it expires !

Your theoretical exam must still be valid to do so
(ie the exam must have been taken less than 3 years ago)

You'll need full details of the person(s) who will accompany you (Begeleider) - full name, ID card number, driver's licence number
If you live in Flanders, they MUST have taken the "Vormingsmoment" course for begeleiders before that !
